The Social Worker is responsible for case management for all residents, including but not limited to counseling services to assist the residents in mentally preparing for life changes. The Social Worker is also responsible for family counseling and coordinating services with external agencies, as well as conducting psycho-social assessments for all residents to ensure appropriate behavioral modifications.
The behavioral modification program is an individualized education program which is designed to help the resident successfully complete all aspects of the 12-week program. The Social Worker works closely with the Job Specialist to encourage retention while participating in the program, and assists with discharge planning to ensure residents have the support they need to execute life changes after being discharged.
